Membership Application

CPE offers two Categories of Membership (General and Organizational).  
General membership is for individuals who are not part of an organizational membership.


Organizational memberships are for organizations that are involved in the assessment and enhancement of physicians in practice.  Organizational members may send up to five individuals to CPE meetings at the member rate.
*Please see Organizational Member criteria. Applicants for Organizational Membership must complete and submit the “Statement of Organizational Activity” within the application form.  When completing the application form, please allow sufficient time to answer a number of questions related to the member criteria.  Applications will be reviewed by the CPE Board of Directors prior to acceptance as an organizational member. Organizational membership applications are reviewed by the Board once per quarter. Potential organizational members that would like to apply in time to receive a discount on the fall meeting should apply by August 16th to give the Board time to review their application. Applications received after August 16th may not be considered prior to the fall meeting.

*Please Note:  Membership is done by calendar year and
all memberships will expire on December 31st regardless of when you registered.
